Liability Waiver & Release of Claims
Mid-South Basketball Association Tournament

1. Assumption of Risk
I understand and acknowledge that participation in the Mid-South Basketball Association Tournament involves physical activity, including but not limited to basketball games, warm-ups, practices, and related events. I recognize that these activities involve inherent risks, including but not limited to bodily injury, illness, permanent disability, paralysis, or death.
I knowingly and voluntarily choose to participate and assume all risks, whether known or unknown, associated with participation in the tournament.
2. Release and Waiver of Liability
In consideration for being permitted to participate in the Mid-South Basketball Association Tournament, I knowingly and voluntarily release, waive, discharge, and covenant not to sue the Mid-South Basketball Association Tournament, Malik Hines, Micheal Carter, event operators, volunteers, officials, sponsors and partners (including Updated Spaces of Mississippi/Momentum Logistics), venue owners, and all affiliated parties from any and all claims, demands, causes of action, or liabilities of any kind, including claims arising from negligence, that result from or are related to participation in the tournament.
This release applies to any injury, loss, or damage suffered by me while participating in or attending the event.
3. Medical Responsibility
I certify that I am physically and mentally capable of participating in this event and have no condition that would prevent safe participation. I understand that the tournament does not provide medical insurance coverage and that I am solely responsible for any medical expenses incurred as a result of participation.
I authorize tournament staff to obtain emergency medical care on my behalf if deemed necessary and accept full responsibility for all associated costs. I understand that no medical personnel may be present at all times and that participation is at my own risk.
4. Code of Conduct & Eligibility
I agree to comply with all tournament rules, eligibility requirements, and codes of conduct as established by the Mid-South Basketball Association Tournament. I understand that failure to comply may result in disqualification or removal from the tournament without refund.
5. Media Release
I grant irrevocable permission to the Mid-South Basketball Association Tournament and its affiliates to record, photograph, and use my likeness, image, voice, and participation in any photographs, video recordings, or other media for promotional, marketing, broadcast, or commercial purposes in any medium, without compensation.
6. Binding Agreement; Governing Law
I understand that by registering a team, submitting a registration form, or participating in any capacity, I am entering into a legally binding agreement.
This Agreement shall be governed by and construed in accordance with the laws of the State of Mississippi, and any legal action arising out of or related to participation in the Mid-South Basketball Association Tournament shall be brought exclusively in a court of competent jurisdiction located within the State of Mississippi.
7. Severability
If any portion of this Agreement is found to be invalid or unenforceable under Mississippi law, the remaining provisions shall continue in full force and effect.
8. Acknowledgment
I affirm that I am at least 18 years of age, or I am registering on behalf of a team whose participants meet all eligibility requirements. I have read this Liability Waiver in its entirety, understand its legal effect, and voluntarily agree to be bound by its terms.

Agreement to this waiver is required during team registration. Submission of the registration form constitutes acceptance of all terms outlined above.
